Requisition Comments and Notes
POSITION DESCRIPTION** Facility is no longer accepting candidates within a 50-mile radius. Do NOT submit candidates with a permanent address within 50 miles of the hospital. ALL RTO requests must be in the packet at the time of submission. Anything sent after, will not be approved** OR RN needed for Shock Trauma OR, Looking for 2+ yrs of experience in trauma, complex ortho and neuro. CNOR preferred. Must be able to circulate and scrub trauma cases for Ortho, ENT, Plastics, Vascular, Neuro and Wound cases. Min of 36 hours in 6 weeks of on-call required, Min 1 weekend per month and Holidays required. BLS required, must be in packet at time of submission. Must have active ORT certification in packet at time of submission. COVID vaccine required. ONLY Medical Declination accepted, Must have signed and authorized documentation from Physician in packet or submission will be dropped. UMMS does not accept Religious Declinations. Flu Season has ended at UMMS and vaccines are no longer required. REQUIRED: Must provide Circulating and scrub skills checklists in packet.

Required Qualifications
Years of Experience: 2. Perm MD RN Lic or compact state lic
BLS
# of beds and trauma level after every facility name listed in the employment history.
OR SCL and Reference within a year
COVID Vacc or Medical Declination (to be uploaded with the file)
50 mile radius rule
All RTO must be noted at the time of submission
