Nurses Working Together to Build Camaraderie

I’ve been visiting my adult daughter for the past week or two in-between contracts.  She works night shift and when she got home the other morning; she left me a note that she was going to have an after-work breakfast with some of her co-workers.  When I woke up and found her gone and read her note; I became nostalgic.

Years ago, I worked a factory job on night shift.  We worked 8 hour shifts and 2 or 3 mornings after work, we would all go to the local diner and enjoy coffee and a bite to eat together.  We discussed work, our families, problems and joys.  We built a camaraderie by doing this that extended into our work hours.  I feel that this made us more productive and helped us work together a little better as a team.

I miss this camaraderie.  Nurses barely talk to each other during work hours, cut down each other and as many nurses state; we “eat our own”.  I have yet to figure out why we do this or how to fix it.

I recently read an article written by Lisa Gonzales, RN, who is President of Absolute Nurses.  Why Nurses Should Socialize Outside of the Workplace is an excellent article and may explain some of the reasons that we as nurses do not socialize outside of work.

…sometimes it’s just a matter of asking if a co-worker would like to do something after work or on a day off.

Ms Gonzales recommends 10 Things that you can do with your fellow nurses.  In addition to her suggestions, I would like to add a few of my own.

– Invite your co-workers for a weekend cook-out.
– Organize a carry-in lunch on the unit.
– Take cupcakes, cookies or a cake to work once a month to celebrate the Monthly Birthdays (suggest that this is rotated between the nurses on the unit).
– Get involved with a fund-raiser  for a good cause and invite your co-workers to join you.
– Find out what your co-workers interests are and find something in common.

Remember that sometimes it’s just a matter of asking if a co-worker would like to do something after work or on a day off. As a traveler, I do this frequently. During my first week or two on a new contract I will ask the staff what their favorite restaurant is, where they go on their off-time, if they can recommend an activity in the area. When they give me their recommendations, I ask them if they would like to join me. You might be surprised how often they say yes.  So next time, just ask!

Can I Take a Travel Nurse Assignment with a Pet?

Travel Nursing with a Pet can have its own questions and difficulties.

This is one of the foremost things on pet owners’ minds when considering travel nursing. The answer…YES!!

Something to keep in mind is that every pet is individual, and you should look at your pet’s personality, health and safety first and foremost. For example, traveling might be too stressful for an older animal; leaving them at home with a trusted family member might be best. There are pros and cons to travel nursing with a pet, and only you can decide if it’s favorable.

PROS:

  • Having a pet is the same as travel nursing with a trusted friend and companion. It can be helpful to help you get through the loneliness.
  • Having a dog will encourage you to get out of the house and explore. It’s always more fun to explore with a friend.
  • Having a dog is an instant conversation starter. When walking my dog, I am constantly approached and have met some great people this way.
  • You’re never alone!

CONS:

  • Expense. Having a pet is an added expense: deposits, daycare, etc. Most housing options that allow pets require a non-refundable deposit. I have paid as much as $400, non-refundable, to have my furry friend with me. If you change jobs after each contract, it can get costly.
  • Limited options for housing. This is dependent on the area. Some places are much less pet-friendly than others.
  • Increased preparation. If you’re going to take a weekend away and explore the local area, you will either have to plan to take your pet with you or find alternate care. Not only can this be expensive, but you DO have to plan for it.

Beat the Boredom: Travel Nursing Activities and Ideas

“What activities did you do on your travel nursing assignment in ___________?”

You are going to have a really hard time answering that if you don’t get out and explore and see what is out there!

Most recruiters will tell you that their travelers are most happy on assignment when they are out and about and enjoying their temporary home and exploring their community, outside of work. Getting our and about is a great way to fend off homesickness, even for the most experienced #GypsyNurse. Even if you do not know what to do, no worries! We have collected some great ideas from fellow travelers, just like you. Even if you do not have hobbies, or you might be kind of shy- there are still ways for you to get out and enjoy your surroundings. The key is to get out and take that first step.

Beat Travel Nurse Boredom

Here is list of a few ideas that were submitted by fellow travelers. There is sure to be something that will appeal to you.

  • Meetup.com – a website dedicated to helping people in a specific geographic area meet up, get together, and plan outings or get togethers.
  • Local parks- these are great places to sit, relax, get some fresh air, maybe read a book. Maybe hike the trails and enjoy nature (pick up a book at the library on local birds or wildlife)
  • Bike rentals- great exercise alone or with a group
  • Ask the locals- a great conversation opener with coworkers. Ask them what they like to do for fun, and then invite them along!
  • Geocache- fantastic way to get out and adventure, locating little hidden caches that are placed in specific areas. Geocaching.com has a great explanation, and list of caches by city.
  • Google the ‘Top 10’ things to do in the area- great way to get local recommendations on restaurants, festivals, historic locations, etc.
  • A lot of people like antiques, or second hand stores, etc. Spend a day off exploring the shops and you might find something unique to remind you of your assignment to take home.
  • Chamber of Commerce- this is a great resource for local activities. For Example: strawberry picking, peanut festival (Virginia)

Are you stuck in your comfort zone?
Gypsy Nurse Ambassador, Patricia Carter said, “Some people are simply afraid to get out of their comfort zone. The activities, events & festivals are there, you just have to want to get out & explore. What I do varies with location. I am on the East Coast. I am a Ballroom Dancer, I don’t leave home without my dance shoes ! I ask the Nurses I work with that live in the area…. best recommendation! They can tell you where not to go! There are day City Tours , Hiking , Dance Studios, Restaurants. I find out if there are other Travelers in the hospital & organize a Meet & Greet.”

Check the Network group on Facebook for local Meet & Greets! If you do not see one posted, create one! Baseball games, football games, museum tours, etc. There are so many things to go and see and do! The key is getting out and doing them! Make memories on your assignments. If you like taking pictures then go explore your surroundings, and take your camera with you. You never know what you may find.

Suggestions, Suggestions

  • TripAdvisor.com has a list of free things to do in just about any area.
  • Check out the state/city website. Most have a website with a list of local free or “Under $25” activities.
  • Ask your waitress, or bartender what they recommend. They can be a wealth of information on fun, interesting things to do in the local area, or within a short drive.
  • Apps, Apps, Apps- one overwhelming response from fellow travelers was see what apps are available for a specific area. They are usually loaded with great ideas on what is going on around you.

As active as our jobs are you would think that we get enough exercise, not necessarily, and the more active you are, the healthier you are. Look up local running or walking clubs. You would be surprised how many people get together and just walk, or even go running together. Check out the local YMCA for Zumba classes, yoga, swimming, or dance classes.

If you are fortunate enough to be in a coastal area, why not try snorkeling, scuba diving, or even surfing. Community centers are a great place to learn arts and crafts, painting, pottery, even take a photography class.

Another great suggestion is check out Groupon or Living Social for local activities that are going on or coming up in the area, or within a short drive. Better yet, get a group of other travelers together and make a day of it.

In the winter months you might be a little more limited, but you can still get out and enjoy yourself. What a perfect time to take a beginner skiing class, or even snowboarding! Check with the locals! They can tell you the best non-touristy things to see and do. Ask around at work- several of the travel nurses on the Network ice skate and the first thing they do is search out the local rink. 

Regardless of what you choose to do for fun while you are on assignment, make sure you get out and explore your surroundings. You never know what is out there if you do not get out and go!

Nursing: A Healthy Back

A nurse’s back is his/her most used tool. If you end up with an injury or a strain, it will make it almost impossible to do your job and take care of others.

We push, pull, lift, turn all day long.  It’s important to maintain good back safety.  Back injuries are very prevalent in the nursing field.  In the latest study that I can find, the Bureau of Labor Statistics states the following:

In all industries combined, 1998 injury data shows that nearly 12 out of 100 nurses in hospitals and 17.3 out of 100 nurses working in nursing homes report work-related musculoskeletal injuries, including back injuries, which is about double the rate for all industries combined.

It’s important as a nurse to remember to take care of your back.  Here are a few reminders of ways to help prevent back injuries.

  • Before you lift, turn, or transfer a patient, think through the task.  Do you need assistance? Is there equipment that will help?  Are you able to maintain both your own safety as well as the patient’s safety?
  • Utilize proper lifting techniques.  We have all been taught safe lifting techniques; use them.
  • Use assistive equipment when it’s available.  It may take more time, but your health and safety are worth it.
  • Ask for assistance from your co-workers to turn, transfer, etc.  In addition to asking for assistance, be willing to assist your co-workers when they ask.
  • Perform regular back exercises for good back health.
When I think of an exercise routine, I immediately think that this takes out of my daily schedule.  The Mayo Clinic has a quick 15 minute Back Exercises side-show that will help keep your back in shape.

Maximize Your Travel Nurse Housing Per Diem

What do you do when the rental housing market is very tough, expensive, or limited availability?  How do you find something affordable so that you can maximize your travel nurse Housing Per Diem?

One of our Network Group members Amy Sherman RN, recently shared her experience and gave The Gypsy Nurse permission to re-print and share with you here on TheGypsyNurse.com

My $236 Bedroom

Amy attests that “Every single thing in the photo was purchased from Craigslist or thrift stores (including a foam mattress in pristine condition!) This is what you can do when you take an unfurnished apartment and the stipend.”

$164 Living Room

Amy says that it took about two weeks to fully furnish her unfurnished apartment.  “I was on a mission.”  She also convinced her landlord to rent the apartment short term by agreeing to leave the furniture at the end of the lease. It’s a win-win for both the renter and the Landlord.

 “I love hunting down bargains, it’s like a scavenger hunt to me. Plus, I get to see a lot of the city.   – Amy”

What a great way to explore!

Another member travels with an air bed. She uses her totes as nightstands, TV Stand and end tables. Yet another member states that she spent about $80 on furniture and then negotiated with the landlord to pay utilities in exchange for the furniture.

Finding affordable temporary housing can be a very stressful part of this job. To be successful at it you have to have a lot of tricks up your sleeve and know what resources are available. These are just a few examples of some of the resourceful things you can do to make the most of your housing per diem.

Travel Nurse Burnout

I’ve talked in the past about Nurse Burnout being relieved by travel nursing but what happens when the travel nurse experiences burnout from being on the road and away from family for too long?

I’ve recently experienced this ‘new to me’ phenomenon.

I’ll generally take time off Travel Nursing around Thanksgiving so I can spend some ‘holiday’ time at home before the mad rush of the actual holiday begins.  I work the holidays…always have.  It normally doesn’t bother me to be at work on Christmas day, Christmas eve and the new Year holidays.  I take a certain joy in the fact that I can work and allow someone else to take the day off to spend with their family.

This year was different.  I don’t know the reason, I’m still trying to process everything.  The holidays this year for me were lonely and I couldn’t wait to get home and spend time with my family.  I think I was experiencing Travel Burn Out and I’ve learned some lessons along the way.

Suggestions to Eliminate Travel Nurse Burnout

  • Know what ‘family’ events are important in your life and KEEP them Important.
    • For me, this was an awakening.  I always knew that going home before the holidays was important, I’d always planned around it in order to be able to. Neglecting this time with my family was detrimental to my mental well-being.  It’s not always about the physical health but also your mental health. If you can’t go home to the family bring the family to you!
  • Take time off from Traveling and Re-connect with family and friends.
    • Nursing alone is a stressful job that we need to take time away from to hold onto our compassion.  I’ve found that making sure to take time to do the things that I enjoy with people that I enjoy being around has made me a more caring nurse.  I was recently on contract and had just come back from a short hiatus. My co-workers were stressed, and exhibited a lack of patience with the patients.  I’ve experienced these same episodes in the past but this time was different.  Having had taken time off to re-connect with ME, I felt I was able to truly take better care of my patients. I was happier. I was less frustrated with the hospital and staff inadequacies.
  • Know that it’s ok to take a break from being a Tourist
    • While working in a new city one of the great things is that we get an opportunity to be a tourist.  In any given area, we develop a list of ‘To-Do’ things that we want to see, do, or experience in our short time.  I find myself occasionally feeling guilty for the day off spent lying on the couch catching up on a favorite TV series or simply taking a day out to rest and relax.  Feeling like you have to spend every waking moment (when not working) out exploring and checking things off your list can become almost a chore.  Remind yourself it’s ok to take a break. Another option is to combine tourism with something that you enjoy. These running tours are the perfect example (if you are a runner).

Top 10 Travel Safety Tips

Safety is of great concern to travel nurses.  Here are my Top 10 Travel Safety Tips.

Please note that this list is not all-inclusive.  Most generally, if you exercise common sense and use your ‘gut’ feelings you will be fine.  The biggest thing that I will stress is that if you feel unsafe, leave the unsafe location immediately. Always be aware of your travel safety.

Here are some tips that I try to follow to stay safe:

1. Check the new neighborhood:

2. Plan your stops in town vs rest areas.
Avoid places without vehicles or people around.  I prefer truck stops as they are usually well-lit and active.  I park close to the door and if after dark, in a well-lit area.

3. At your assignment location, drive around and orient yourself during the day.
Scout out grocery stores, the hospital and other things you need to visit while it is daylight, so you don’t have to be wandering around in the dark looking for them.

4. Have some sort of Emergency Assistance
AAA is a great resource. If the car breaks down or gets a flat, stay in the car until the tow truck (well-marked with AAA) arrives. AAA will generally ask if you feel safe, if you do not feel safe tell them and they will usually send a police officer to you.

Top 10 Travel Safety Tips

5. Give A Friend or Family Member your Itinerary and check in.
I give a basic itinerary (route) as well as calling to check in when leaving and arriving.

6. Select your Hotel with Safety in mind.
I suggest using a hotel with rooms on the inside, i.e. no door straight out to the parking lot

7. Choose the 2nd Floor
Always ask for a hotel room on an upper floor (2nd or higher), as ground floor rooms are the most vandalized. This is a good tip for any temporary housing as well.

8. Never travel without at least two sources of money available.
In addition to whatever cash you have on hand. Periodically you might find that your bank thinks your debit or credit card is being used suspiciously (has happened to me) and freeze it on you without warning. Don’t store these all in one place.

9. Secure any belongings that stay in your vehicle overnight.
Don’t leave anything obviously open to be viewed from the windows.

10. Always park in a well-lit area.
If I am able to park in a location that can be viewed from the front desk all the better.

Why Nurses Eat Their Young! – A Nurse Perspective

After reading The Gypsy Nurse’s Post Nurses ‘Eat Their Young?’, I wondered if anyone had a different opinion than me on the cause of this phenomenon.

A nurse “eating their young” is an almost accepted form of bullying by many nurses (the ones that participate). Most don’t admit (even to themselves) that they are eating their young but cleverly try to disguise their bullying actions by positioning their attitude/behavior toward the new nurse as “helping” or “providing an educational opportunity.” People who have worked in other careers agree that they have encountered this phenomenon in other jobs. However, they are shocked at the prevalence that it occurs in nursing.

Personal experience

Indeed, I have personally experienced it and seen it done to others. So the question is, “Why would educated and professional nurses engage in such poor behavior?” I believe there are two main reasons this attitude prevails in nursing. The first reason is… High school never ends! Refer to the subject of Social Psychology. Social Psychology attempts to explain behavior such as cliques and groupthink.

Cliques and Group Think are the underpinnings of the complicated social culture of high school and most workplaces.

So when nurses form a clique that accepts it is the “cultural norm” to eat their young, even those who don’t agree to keep quiet, not to upset the status quo. The irony of it all is that we are consistently voted America’s most trusted profession. Yet, we can not even be trusted to treat each other with kindness and respect. It is really quite sad that this shallow Group Think occurs among such talented and educated people!

What spurs this to become the social norm

So the question remains, “What spurs nurses to allow this to become the social norm in their group?” This brings me to my second reason for such an attitude to prevail in nursing. The profession of nursing in our society is synonymous with sacrifice. This sacrifice starts in nursing school and continues into the workplace.

Think back to nursing school. First, nursing program acceptance can be similar to the rush of winning the lottery because it is such a competitive process. Then you realize that your nursing professors are on a mission to “weed” people out and make sure only the strongest students pass their classes. They want to eliminate the “weak links” because nursing is a high-stress, academic, and moral science (and the school’s pass rates are public for the entire world to see). As a nursing student, you begin to study with intensity, stop socializing, working out, and attending family functions. But it is all good! You rationalize that nursing school doesn’t last forever, and it is worth the ….SACRIFICE!

You pass NCLEX and breathe a sigh of relief.

and say to yourself, “I can live again!” Then you begin the long struggle to find a job. But wait! You finally find a job and discover it is in the specialty you are not interested in, and you will have to work nights. Again, you encounter SACRIFICE! But hey, I have a job to pay bills (often large student loan bills).

Some nurses discover that adjusting to the night shift is impossible. The longer they stay on nights, the more dysfunctional their life and sleep become. If you have young children, they make comments such as “All you do is sleep.” You and your children are on opposite schedules, so to them, it seems like you are sleeping their lives away. Also, you work weekends, odd hours, mandatory overtime, on-call, and miss holidays/special events with your family.

Furthermore, some patients and their families can be complicated and unappreciative even when you do the best you can with poor staffing ratios. Not to mention you’re often dealing with burnout if you are working in a high-stress critical care area. By the way, you have not eaten, drank water, or went to the restroom during most of the shifts you work. Don’t forget the nurse bullies! The years pass, and you lose track of the numerous sacrifices you have made in the name of nursing.

Erosion of empathy and self-confidence

So, the continuing atmosphere of sacrifice can contribute to the erosion of your empathy and self-confidence. Then enters a new nurse when stars in their eyes about the wonderful profession of nursing. You are unhappy with your situation and treat the new kid on the block just like you were treated; badly in the name of “education.” Or maybe you are a competitive type of person and want to maintain your status in the workplace you have earned. Or, unconsciously, you perceive a new nurse as a potential future threat. So you decide to test the new nurses and see what they are made of. Let’s mold a new nurse into a super nurse of steel!!

I think this is a general scenario of how nurses come to engage in nurse bullying.

Admittedly, some work environments are more accepting of new nurses than others. I realize that my example borders on the dramatic. Or does it? I have informally interviewed a large number of nurses during friendly conversations; the recurring theme is usually sacrificed. Sacrifice is the fertile ground that bullying behaviors come to seed in.

The nursing profession has decided to combat nurse bullying is similar to the approach we take in preventive care, awareness, and education. Nurse bully experts are focusing on the workplace. All over the country, hospitals are waking up and requiring education for their staff to prevent nurse bullying in the workplace.

However, since you can not control others’ actions, only your own nursing education specialty needs to step up to the plate. Nursing schools have just as much responsibility as the workplace. They have a moral obligation to help students pass the NCLEX and prepare them for workplace success.

Nursing schools should address, in their curriculum, this phenomenon and the tactics that new nurses should employ to fend off the “wolves.” Self-confident new nurses armed with the knowledge and concrete actions to defend themselves could be a deterrent for bullies. At least they won’t be blindsided when entering the workforce. We need happy nurses who want to stay in the nursing profession to care for patients and deal with the changes in healthcare.

Violence in the Workplace: A nurses perspective

Violence in the workplace is not tolerated. Most professions will protect their workers from workplace violence.  Nursing, however, is very different.  How can you be protected from the violence that confused, drugged-out, neuro-damaged patients will ultimately give you?  You can’t.  It’s a simple fact that nurses will be assaulted at one time or another during your career.

The National Institute for Occupational Safety and Health (NIOSH) defines workplace violence as any physical assault, threatening behavior or verbal abuse occurring in the workplace. Violence includes overt and covert behaviors ranging in aggressiveness from verbal harassment to murder. (NIOSH 1996)

Violence in healthcare

Violence in healthcare is such a formidable issue that OSHA has put together a complete policy to address it.

Head Nurse, a fellow nursing blogger posted about an incident that occurred to her in the workplace.  “Well That’s Never Happened Before” She puts a funny spin on the experience but the reality can be scary and potentially dangerous.

I’ve been assaulted on many occasions as a nurse.  Once a young confused patient grabbed my stethoscope (that was around my neck) and proceeded to nearly choking me to unconsciousness with it.  Needless to say, I never wear my stethoscope around my neck anymore.

I’ve been punched, kicked, spit on, pinched so many times that I can’t even count them.  This is just the physical violence that I’ve encountered.  I haven’t even mentioned the verbal assaults that I’ve received not only from patients and family members but doctors and co-workers as well.

According to the Bureau of Labor Statistics, the healthcare sector leads all other industries, with 45% of all nonfatal assaults against workers resulting in lost workdays in the US. (BLS, 2006).

Why aren’t nurses and healthcare professionals protected better?  We aren’t allowed to utilize restraints except for extreme circumstances and at that, it takes a ridiculous amount of steps to obtain. It’s unethical to simply ‘drug’ a combative patient.  What is being done to protect us as nurses from violence?
