Or they just want to choose a winter assignment in a new part of the country.

Get to know the community.

It’s hard to focus on work when your living arrangements are in disarray, and your support network is non-existent. Before you pack the car and go cross-country, your interview with the hiring manager is a great opportunity to get a local’s perspective and insights.

  • What is the community like? For example, is it a good fit if you have a family, love the arts, or like to spend your downtime outdoors?
  • Is it hard to find housing nearby? What neighborhoods would they recommend?
  • What types of activities are popular this time of year?
  • What kind of weather should you prepare for?
  • What popular apps do locals use to find events, meet people, or buy used items?
  • Is there anything that tends to surprise people who are new to the area?

You’ll also want to ask about the cost of living. Rent may not be top-of-mind since travel nursing agencies often arrange accommodations or offer a housing stipend to pay for private housing or a hotel. However, the impact of high grocery or entertainment costs can skewer your budget if you aren’t prepared.

Learn about the work you’ll be doing

Your travel nursing assignment contract should clearly explain your responsibilities and expectations for you as well as the healthcare facility and the travel nursing agency you’re working with. However, since we’re talking about your day-to-day work, make sure you’re clear about the things that are most important to you.

For example:

  • How many hours can you expect to work per week?
  • What will your role be, and what types of cases will you be working with?
  • What is the medical facility’s standard nurse-to-patient ratio?

  • What is the parking situation? Is it available for free or at a cost? How far away is it?
  • What are the uniform requirements? Are scrubs provided?
  • Is there a cafeteria or kitchen available for meals?
  • What is the facility’s policy for breaks? And do nurses typically take their breaks?
  • How is the scheduling done?
  • Are travelers often floated? If so, to what units?

Make sure your requirements are covered.

Dealing with difficult patients is a skill. Whether you are new to the nursing scene, or a seasoned vet, you know that there are just some patients that push you a little (or a lot) over the edge some days. Most of the time, we are pretty easy going and know to expect the unexpected, especially if you are an ER nurse. Here are tips for dealing with difficult patients while travel nursing that you can think about when you or your patient are having one of those days.

Listen

Whether your difficult patient is in for something minor or something major, sometimes they just want to be heard. Just a moment to make eye contact and hear them out with all their complaints, doubts, anger, etc. This could be a way of expressing how nervous or scared they are about what may or may not be happening. One of our Gypsy’s put it best on Facebook, “Listen, and listen some more. If a patient has a complaint about a previous nurse, often better to just listen, then obtain the presence of the charge nurse or supervisor, or if the facility has a patient liaison or representative. A lot of times, they just need to be heard. Never make excuses.” 

Respond

It’s typical after a long day or even a long week to have a short fuse. Nurse or not, we are all guilty of this from time to time. Although for you, it may seem like a no-brainer, sometimes all they need is for someone to hear them out and respond with patience and kindness. Assure them that they will have all their questions answered and even ask if anything needs to be clarified. This may take a bit longer for some, but in the long haul will result in a better, more satisfying stay for both.

“Ask when you can’t find something, ask when you don’t understand them, ask for help if you need it, ask if you can help them, ask the patient questions, it’s okay to show emotion to your patient, it’s okay to sit and cry with them. Sitting down next to them for 30 seconds makes it feel like you spent 5-10
minutes with them sometimes. Share yourself with patients and their families.” This can mean the world to your patients and the lives they touch by a simple act of compassion. Showing the patient that you’re human, too, with a little bit of humility can go a long way in calming them down as well.

Kill them with kindness

Biting your tongue can be hard. It’s not always going to be a difficult patient that is just hurt or confused, or frustrated. Sometimes you just plain cannot calm them down or talk sense into them. Unfortunately, sometimes it’s perhaps something they’ve ingested that is making them act that way. For all involved, at times, it may be best for the sake of your own sanity to kill them with kindness. Even at your highest stress level, at least you can leave with no remorse in your actions and may even be able to get a laugh out of it down the road.

If all else fails, be honest

When you have tried and tried, and nothing seems to work, be sure you abide by the policy and let the patient know, calmly, the consequences of their actions if they proceed. Making sure you have documentation of what you’ve done or tried to do, as well as any documentation they may need to fill out to leave AMA, etc., it’s best to have all your ducks in a row. It may be that “gentle” nudge to show them that you care, but respect is expected both ways.

There are many variable when it comes to the best states for travel nurses to obtain a license. Some states like California, always have openings. Others are somewhat seasonal – bringing in travelers during high seasons. Here are 2019 Nursing Trends that might be helpful in determining where to go this year.

Personally, I recommend only obtaining a license for those states in which you have a job offer for.  If you are licensed in a compact state and eligible for compact licensure, your choices are much more vast. There are also seasonal shifts in high-demand locations like Arizona, and Florida. If you are interested in working seasonally in these locations, you might consider obtaining a license in advance for these states.

What Is A Tax Home As A Travel Nurse

After a day of managing potent medications on a critical patient, one would think that unraveling the concept of tax home would be an easy task. Unfortunately, the concept of a tax residence is very similar to an ACLS algorithm which few of us actually master unless we routinely manage codes or are lucky enough to have extra room in our scrub pockets to carry around an ACLS flow chart. So what really is a tax home?

The Complexities of Tax Homes

The complexity of a tax home determination is mind-boggling. However, it is the litmus test of the taxability of tax-free reimbursement payments.

The confusion is exacerbated by agencies, recruiters, and executives who only know part of the rules. Like a patient that knows enough about a diagnosis to be dangerous, players in the healthcare staffing industry are just as suspicious. Unfortunately, the same applies to many tax professionals who stumble over the rules with equal blindness.

The following discussion will just address the foundation of a tax home. There is no one thing that settles the issue. Just like the ACLS algorithm. The determination of tax home goes through many decision points governed by unique facts and circumstances.

Starting point: A tax home and a permanent residence are NOT the same things!

What? What do you mean? All the agencies want to know where my permanent residence is.

This is where most of the confusion over a tax home starts. These terms are, unfortunately, used synonymously by many in our industry.

A permanent residence is a legal concept. Ties that bind you to an area all contribute to the location of your permanent legal home. These include driver’s licenses, car registration, memberships, where you get your mail, the home state of your professional practice license, etc. This does not rise to the level of a tax residence and initially has NO impact on it.

A tax residence is defined by the IRS as one’s principal place of business, which is a loaded term that basically means the area where one makes the majority of their income. It is not where you live.

This is why it is better called an economic home. Most people work where they live. Hence, their permanent and tax residences are in the same place, which explains the synonymous use of the terms. However, many people do not work where they live. Some have more than one permanent job, seasonal jobs, or commute a significant distance to a main job. The definition of a tax residence for these individuals is no different. The tax residence is still the location where the individual makes the majority of their income in relation to the other places of work.

Travel nurses occupy a different sphere. Because their work is mostly temporary, they do not have a primary place of business or income. Since they are in constant motion, never stay in one place for more than a year. The tax code has recognized that it would be unreasonable to expect these individuals to actually move their residence to a different location with each assignment. Travelers with tax homes are never moving. They are mobilizing. The difference is those terms are important as moving involves a change of residence while mobilizing is more of an accurate description of someone who is temporarily away from home.

How do the tax rules address the travel nurse?


For true “travelers,” as defined above, the tax rules allow an exception to the tax home definition. Instead of looking at the primary place of income/business, it allows the tax home to default (fall back on) the permanent residence. For this to apply, however, the travel nurse must meet 2 out of 3 of the following criteria.

  1. Does the individual have significant income at home?
  2. Does the individual have substantial expenses maintaining their primary residence that is duplicated when on assignment?
  3. Has the individual abandoned their historical place of lodging and work?

Most travelers do not work at home (Criteria 1). This means that the balance of travelers must satisfy the second and third criteria to have an acceptable tax residence. (Criteria 2) They have an apartment or house that they own, duplicating these expenses when away from home on assignment. Further, their home is in the same area that it was when they started their traveling career, or they established an income base in the area before traveling (Criteria 3). Some travelers keep regular jobs at home (Criteria 1) and return home on a regular basis between contracts. If the income earned from this job is significant, the requirement of a financial obligation for a residence is not as critical since they are still satisfying Criteria 1 and 3.

1. Keep Yourself Hydrated

Keeping yourself hydrated is very important. If you enjoy coffee, tea, or alcohol on flights, make sure you ask the air hostess for a bottle of water instead of a cup each time. That will ensure adequate hydration.

2. Carry a Bottle of Eye Drops

Try to carry a bottle of eye drops along with you whenever you travel, but make sure only to use eye drops that a doctor has prescribed. If you’re traveling with a child, you may want to consult your doctor if he suffers from the same.

4. Switch to Glasses

Switch to glasses while flying, as contact lenses may increase the chances of dry eyes and hurt your eye health. Moreover, glasses are much easier to nap with than contact lenses if you’re watching many media.

5. Switch Off Air Conditioning

Switch off the air conditioning vent above your seat if you’re flying to prevent dry air from blowing directly onto your face. Since the cabin climate is fairly low, doing away with the extra air conditioning can give you some relief from dry eyes.

6. Give Those Eyes Some Rest

Give your eyes some rest, and try to avoid spending too much time on digital screens. Get up, move around, meditate for a few minutes, close your eyes for a while, and think of what you will do when you land.

7. Blinking Exercises

Blinking exercises not only relieve you of dry eyes but also alleviate headaches and blurry vision.

With all the precautions that you can take, some airlines aim for a more comfortable cabin climate since passengers travel frequently and for long hours. Moreover, check with your airlines about regulations on carrying liquids such as eye drops in your handbag. Keeping one with you would assure a pleasant travel experience.

  • People above the age of 50 are generally more prone to suffer from dry eyes.
  • If you’re taking some medication, check with your doctor if one of its symptoms is dry eyes.
  • Pregnant women or women undergoing menopause can experience dry eyes
  • Wearing contact lenses

ANSWER:

Sure! Obviously, the needs of the hospital and the skill set of yourself and your friend dictate whether this will work. I’ve seen travel nurse friends traveling together in a variety of ways:

  • working with the same company at the same hospital on the same/different units (dependent on individual specialties).
  • traveling with Different companies at the same hospital.
  • different companies at different hospitals.
  • Traveling and working at hospitals in nearby cities and living together.

Additionally, I have seen lots of cases where husband and wife teams take assignments together. This provides an opportunity to travel to new and interesting places they may have never dreamed of living and working.

There are many ways to manage traveling together. Some things to consider when traveling with a friend (or travel nurse spouse):

  • It can sometimes mean taking a contract in a location that wasn’t your first choice.
  • Finding a hospital that needs two travelers with the specific skills/specialties that you and your friend have can be a challenge. Be Flexible!
  • Communicate your needs to your recruiter.
  • Housing and tax issues can be complicated when traveling with another traveler. Make sure to educate yourself on the tax implication.

Most importantly, flexibility and patience are key!

Ultimately, travel nurse friends traveling together can be a wonderful way to see the country and explore your new city.

The Altruistic Nurse vs The Career Nurse

Is there a requirement for a nurse to have a ‘calling’ or to be altruistic in their reasons to become a nurse?

I’ve recently ran across several articles and nursing forum posts related to nurses having a ‘calling’.  Something deep seated in their personality or their mind-set, a need or want to ‘help’ others. Something altruistic in their reasoning for becoming a nurse. The argument that I’m seeing repeated over and over is that you ‘have to’ or ‘should’ have a need, want and desire to help others in order to be a good nurse.

I’m going to rant on this…please do not send me ‘hate’ mail.  If you would like to have a respectful discussion; I’m open and willing to participate

Confession:  I never wanted or desired to be a nurse.

 WHAT??!!??!?

That’s right.  I started my nursing career as a career choice and a JOB.  There wasn’t any deep seated desire to make a difference in the world.  No passion to ‘help others.  No ‘calling’ from a higher power.  Nursing to me was a stable career with attainable educational requirements, decent pay and job security. I consider myself an altruistic person but that was not my drive to become a nurse.

There are many that will immediately think that I am a bad person or worse yet, a bad nurse for my decision to become a nurse as a means to provide myself and my family a decent lifestyle.  To them, I say ‘YOU’RE WRONG’.

 I look at nursing as I would any other job or career that I could have chosen.  I WANT to do a good job.  I need  to perform well.  I expect to be able to take a certain amount of pride in a job well done.  These feelings are no different than if I had chosen to be a basket maker.  If I were a basket maker…I would still WANT to do a good job, I would need to perform well and I would expect to be able to take a certain amount of pride in a job well done.

So, am I automatically a bad nurse because I chose Nursing based on purely career and stability oriented reasons?  Or is it possible that even though my reasons were not altruistic, I am still a good nurse, a patient care-giver and able to provide safe care with positive outcomes?
