JOB DESCRIPTION\nLicensed Practical Nurse (LPN)\nPosition Description:\nA Licensed Practical Nurse is a professional member of the health care team who provides skilled\nnursing care to clients under an established Physician Plan of Treatment, in compliance with the Nurse Practice Act, and\nadheres to the policies and procedures of this agency. This position reports to the TLC Nursing Supervisor. In an\ninstitutional setting this person reports to designated person.\nQualifications:\n1. Must be able to lift, push and pull at least 50 lbs.\n2. Must possess a valid current nursing license under the State Board of Nursing.\n3. Must have a minimum of one year experience in an acute care setting.\n4. Must possess and maintain current cardiopulmonary resuscitation certification.\n5. Must be of mature and responsible character.\n6. Must possess current knowledge of the disease process; emergency interventions and health care measures\npertinent for each individual client.\n7. Must possess good observational nursing judgment and effective communication skills.\n8. Must be of good physical and mental health.\n9. Must complete this agency employment process and orientation.\nDuties:\nThe duties and responsibilities include, but are not limited to:\n1. Provides skilled nursing care.\n2. Observes and provides ongoing assessment of client and family circumstances.\n3. Communicates client changes and needs to the physician and Nursing Supervisor.\n4. Initiates preventive, rehabilitative and therapeutic nursing measures.\n5. Teaches safety precautions; medication actions and interactions; appropriate health care measures, appropriate\nrehabilitation procedures.\n6. Administers medications, treatments and other modalities as ordered by the attending physician.\n7. Maintains current skills, knowledge and practice specific information by completing continuing education\nprograms.\n8. Documents accurately and submits timely nursing notes and reports, according to agency and facility standards.\n9. Works in cooperation with facility administration and personnel.\nPerformance is deemed competent when the Nurse:\n1. Demonstrates ability to provide nursing care within the parameters of the State Nurse Practice Act and the\npolicies and procedures of this agency.\n2. Demonstrates ability to initiate and implement a realistic care plan.\n3. Demonstrates ability to continually assess a client’s changing physical, emotional and social condition.\n4. Demonstrates effective communication skills.\n5.
